Blue Knights CA 1 Procedures for Joining

1. Download the Membership Application here.
2. Have your Club Sponsor complete and sign the approriate section on page 2 of the application.
3. Along with the Application include a copy of your Agency ID card, a copy of your Separation Papers showing years of service and agency, and a copy of your most recent Pay Voucher Statement. (black out all account numbers, Social Security Number and Dollar amounts.)
4. Include a check made out to Blue Knights CA 1 for the amount of $50.
5. If you would like a nametag for your spouse or significant other, include their information and an additional $7.00
6. If you would like your cell phone number listed please include that also.
7. Mail completed application package to:
